Parents: Complete a Health Care Proxy For Your College-Bound Son or Daughter

A Health Care Proxy is a medical power of attorney that allows the person appointed by the student (presumably the parents) to be their health care agent. The appointed agent is authorized to make medical decisions on the student’s behalf if the student is severely injured or rendered unconscious.

Complete Advance Health Care Directive For Your College-Bound Student

Parents should complete an Advance Health Care Directive for their college-bound child if that child is 18 years of age or older.  An Advance Health Care Directive is a medical power of attorney (or healthcare proxy) that allows students to appoint a person as their healthcare agent; the designated healthcare agent (Mom and/or Dad) is authorized to make medical decisions on behalf of the student.
